COS Van Use for Students
This page is intended for students that need information on driving vans for College of Science (COS) classes.
Faculty & staff may visit Van Use & Defensive Driving for Faculty & Staff.
In order to drive a COS van (which is a state vehicle), you must first be registered with the College of Science and the Driving on University Business (DUB) Program.
Please read this entire web page.
For information on becoming certified with the University Corporation (to drive the Watershed vehicles or on grant-related work), please visit the Corporation Defensive Driving Program website. Please note that the University (Stateside/DUB) does not accept the University Corporation driving documentation, including their defensive driving course. However, the University Corporation does accept the University (Stateside) defensive driving course. If you will be driving for both entities, it is better to complete the University (Stateside/DUB) requirements first.
Before being permitted to drive a van, all van drivers must have current approved driving status through Driving on University Business, and the College of Science. If you are interested in becoming a volunteer driver for class field trips, please contact the College of Science Defensive Driving Coordinator at cos_defensivedriving@csumb.edu. In your email, please provide the name and section of the class or classes for which you plan to drive.
Keys will not be released to drivers unless all the correct driver paperwork is complete and up to date. Please contact cos_defensivedrving@csumb.edu if you are unsure of your driving status.
Step 1: Person of Interest form
Fill out and submit the appropriate Person of Interest (POI) form (Use one of the POI forms below, and NOT the volunteer form on the Driving on University Business website). Use the form that corresponds to your major department:
Please allow 10 business days for the Volunteer Form to be processed, and before driving a State vehicle.
Under ‘Department Information’ portion of form, please include the following specific information:
Type of Appointment: Volunteer
Affiliation: CSUMB Student
ID#: Student ID
Start Date: today's date
End Date: One year from start date or graduation date (whichever comes first)
Description of Duties: Drive College of Science (COS) vans for COS field trips.
Additional Duties: Check “Yes” ONLY for first question ‘drive a vehicle on university business’.

Print, sign, scan, and submit the form as a pdf to cos_defensivedriving@csumb.edu. You may also turn it in at Room S310, in the Chapman Science Academic Center (bldg. 53).

Step 2: DUB Application paperwork
Submit the following items to cos_defensivedriving@csumb.edu: the DUB Registration form, the INF 1101 form, and a copy of your driver's license. Please contact cos_defensivedriving@csumb.edu for partially filled in documents. The DUB registration form and the INF 1101 are also available on the (DUB) webpage. Instructions for all forms can also be found on the DUB website, but email cos_defensivedriving@csumb.edu if you have questions.
- The CSUMB Driving on University Business (DUB) registration form. (Please check or fill in the bubbles next to Academic Affairs and Student Volunteer.)
- Form INF 1011, Authorization for Release of Driver Record Information.
- A copy of your current driver license (regardless of state), front side only.
Students do not normally submit the "STD 261" form. Please remember to use the Person of Interest form listed above and not the one that appears on the DUB website.

Step 3: Defensive Driving Training course
You will need to self-assign the training in SumTotal. SumTotal will appear as an App on your CSUMB Dashboard, but you will likely not have access until your POI has been processed. Please search for 'Defensive Driving Program' in the search field once you have access. Upon completion of the course, please submit a screenshot of your completion certificate to cos_defensivedriving@csumb.edu
. If you have not been assigned the training after 10 days or have any questions, please contact cos_defensivedriving@csumb.edu All paperwork will be processed as quickly as possible. Please submit paperwork early in the semester to ensure driving privileges.
Step 4: Orientation
Contact cos_defensivedriving@csumb.edu to schedule an in-person orientation. The orientation normally takes about 30 minutes. We will go over information specific to the vans and driving for the College of Science, as well as review some information listed below. If you would like to also schedule a driving orientation, please mention that in your email.
Paperwork in vans
- Fill out the Daily Operator's Checklist before you leave. For safe usage, all vehicle functions and safety features including: brakes, lights, tire pressure, horn, gas, oil, First-Aid kit, and fire extinguisher should be checked before leaving on your trip.
- Fill out the Monthly Travel Log (STD 273) each time you drive a van. You must enter: the date; initial and final odometer readings; total trip miles; origin and destination; time; and print your full name (not your initials or signature). Two lines on the log should be used for each trip - one for departure and one for return.
Safety
- The driver and all passengers must wear seat belts and shoulder harnesses any time the van is in motion.
- If you will be traveling to a remote location, it is advisable to take a radio or cellular phone along in case of emergency.
- Remember: off-road travel in COS vans is strictly prohibited!
- If you have an emergency, you will need to contact your Instructional Support Tech and the Science Operations Coordinator (Margaret Geissler, 831-582-4213).
Refueling
Each van has a State gas card attached to the key chain. Each van must be refueled with its own gas card; if you have multiple vans out on a field trip, you must use each van's designated gas card for fuel. A State gas card cannot be used to fill up a private vehicle. Fill the van with gas when the fuel gauge is at or below half full. To see how to use the gas card, click here.
- Gas stations such as Chevron, Exxon, Citgo, Mobil, Shell, Texaco accept the gas card. Make sure that the card is accepted and authorized before you pump the gas!
- Fill the van with regular (87 octane) unleaded gas at self-service pumps. The purchase of premium grade (89 or 92 octane) unleaded gasoline is prohibited.
- No full service is allowed.
- For Enterprise Fleet leased vehicles (#156, 157, 158; Ford Transit): To obtain Roadside Assistance 24 hours a day, call 1-800-450-2500 and select option 4. The Enterprise Roadside Assistance Program provides prompt mechanical first aid, towing, battery service, wheel change, gasoline delivery and lock-out service for your company vehicle.
- For Department-owned vehicles (#36, Ford Econoline): call the National Automobile Club 24-hour roadside service number 1-800-600-6065. They can provide emergency road assistance and if necessary, towing to the nearest state-approved garage.
Accidents
- If you are involved in an accident, remain calm.
- Call the local police or California Highway Patrol to the scene of the accident. Contact the Van Coordinator and the Science Operations Coordinator (Margaret Geissler, 831-582-4213) as soon as possible.
- Do not discuss fault or attempt to determine liability with the other party or representative.
- Obtain the name, address, phone number, insurance carrier, and policy number of all people/vehicles involved. Provide them with the same.
- If safe to do so, take photographs at the accident scene to include damages to the state vehicle, the other parties vehicle/property, etc.
- Fill out the Accident/Incident Report form, form STD 268 "Incident/Accident Report". Within 2 business days, review the information on the "Vehicle Accident Report" with your supervisor/health and safety coordinator and have them sign it. The information on these forms will be transferred later to form STD 274 "State Driver Accident Review – Supervisor’s Review Form", which is to be submitted to several State agencies within 2 business days after the accident.
In each van, there is a binder containing:
- State Vehicle Accident Report forms (STD. Form 269, 270, 274) and SNS Accident/Incident Report forms
- Emergency contact numbers
- State Motor Vehicle Liability Self-Insurance Letters
- Details on how to use the gas card and a list of participating gas stations
