Adobe Sign: Alcohol Approval Form
In order to ensure accuracy and timeliness the Alcohol Approval Form and process will be moved over to Adobe Sign. Adobe Sign is an electronic process, which creates efficiencies in our processes and saves paper. For assistance or guidance in filling out the form and additional Adobe Sign form information, please review our FAQ’s and Alcohol Approval Form Instructions below.
AdobeSign template forms such as the Alcohol Approval form, are templates that are meant to be filled out after it is sent to it's assigned recipients. The individual filling out the form should enter in their email as the "Assigned Recipient" and ensure that they enter the email addresses of all those required to sign then press Send. Please do not send to the Risk Analyst, Risk email, or Defensive Driving email during this process. Once you receive the template in your email, you can then complete the form information. The appropriate recipients have already been added to the template or envelope for you so you will only need to hit Send to complete this process. After you have collected all the required signatures send to the appropriate risk@csumb.edu (Alcohol Approval) or defensivedriving@csumb.edu (
DUB documents) email for processing. NOTE: At any time during this process, the creator/submitter can check the form status: where it is within the process.

Release of Liability/Waiver Release with Photo Release
Releases must be stored for at least three years after an activity ends. Documents signed by a parent or guardian on behalf of a minor must be retained for at least three years after an activity ends or until the minor turns twenty, whichever is longer. Departments or units who sponsor an activity requiring a release should store the release by activity date in alphabetical order by participant name. Doing so greatly expedites retrieval if a release is needed for a legal proceeding. If space becomes a problem, releases can be microfilmed or stored electronically in lieu of the signed original.
You must also have a Release of Liability/Waiver Release (DOC) for each participant; to be carried with the MPP/Supervisor on the date of the event.
