Academic Objective Change Request GR/CR

This form is used to request an academic objective change from graduate degree to credential or certificate and from certificate to graduate degree. Credential students may not use this form to change to a graduate degree.

Additional Unit Authorization UG (online form)

This online form is used to request to enroll in more than the maximum allowable units for undergraduate student levels and majors. Advisor approval is included in the form process.

Additional Unit Authorization PB (for Graduate/Credential students only)

This form is used to request to enroll in more than the maximum allowable units for graduate or credential students only. Advisor or Program Coordinator approval is required. Completed form is submitted to the Office of the Registrar.

Application for Cross Enrollment

This form is used to apply for cross enrollment at a California Community College or University of California campus while enrolled at CSUMB. Approval from CSUMB Office of the Registrar and Host Campus Instructor are required.

Application for Exchange Enrollment (for Graduate students only)

This form is used to request to enroll in courses through CSUMB exchange agreement at the University of California Santa Cruz. Completed form is submitted to the Office of the Registrar.

Application for IntraSystem Concurrent Enrollment

This form is used to apply for concurrent enrollment at another CSU campus. Approval from CSUMB Office of the Registrar is required. If approved, application will be forwarded to Host Campus for approval.

Application for IntraSystem Visitor Enrollment

This form is used to apply as a visitor at another CSU campus for a specified term. Approval from CSUMB Office of the Registrar is required. If approved, application will be forwarded to Host Campus for approval.

Class Time Conflict

This form is used to obtain permission to register for two classes which overlap in scheduled meeting times. Approval from Instructors for BOTH classes are required. Completed form is submitted to the Office of the Registrar.

Course Audit Form

This form is used by students to request to AUDIT a class. Approval from course instructor is required. Completed form is submitted to the Office of the Registrar.

Authorization to Release (OASIS online form)

This OASIS form is used to designate one or more trusted persons—your third-party designee(s)—to receive information about your records when they contact the University.

Graduation Application (OASIS online form)

This form is used by students who need to apply for graduation or change their graduation date to a term that is not currently open online. This form should only be used when an online application cannot be completed. Completed form may be submitted electronically to graduation@csumb.edu.

Incomplete Grade Contract

This form is used to request an incomplete grade. If the incomplete grade request is approved, the form is also used to document the completion deadline for any remaining requirements. Signatures from Instructor and Student are required. Completed form is submitted to the Department Chair.

Late add petition (OASIS online form)

This OASIS form is used to request to ADD or SWAP (change sections of a class only) after the add/drop deadline of a term. All requests are by special exception and subject to approval. After add/drop and prior to census, instructor and department chair approvals are required. For petitions submitted after enrollment census, college dean approval is also required. Completed form automatically routes to the Office of the Registrar. This form is only available after the add/drop deadline of a term.

Leave of Absence Request

This form is used to request a Leave of Absence from the university. For graduate students, Program Coordinator approval is required. Completed form is submitted to the Office of the Registrar.

Petition for Exception UG

This form is used by undergraduate students to petition for an exception to university academic policy. Completed form is submitted to University College for review. 

Petition for Exception GR/CR

This form is used by graduate and credential students to petition for an exception to university academic policy. Completed form is submitted to Graduate Studies & Research for review.  

Reinstatement Petition Grad Cred

This form is for graduate and credential students that have been academically disqualified and would like to petition for reinstatement. Completed form and supporting documentation is submitted to the Office of Graduate Studies & Research.

Repeat Petition UG

This form is used by undergraduate students to request to repeat a course for grade point average (GPA) adjustment and to request to exceed total repeat limits. Approval from advisor is required. Additional approvals vary based on request type, but will include Department Chair and/or the Dean of University College. Completed form is submitted to the Office of the Registrar.

Credit by Assessment Request

This form is used to request credit by challenge exam or prior learning assessment. Completed form and any supporting documentation is submitted to the department from which the credit is requested.

Residence Questionnaire

This form is used to establish residency status or to request a residency reclassification. Completed Residence Questionnaire is submitted to the Office of Admissions. For students requesting a Residency Reclassification, completed form is submitted to the Office of the Registrar.

Residence: CA Non Resident Tuition Exemption AB540

This form is used to request a Nonresident Tuition exemption (AB540). Completed form is submitted to the Office of Admissions.

Student Personal Information Change (online form)

This form is used to update or correct your legal/primary name, Social Security Number, date of birth, or gender. Please note: Active students can update their preferred name and contact information in their OASIS Student Center. If you choose not to (or are unable to) submit the online form, you can submit the paper form to the Office of the Registrar.

Withdrawal Request

This form is used to request to withdraw from one or all courses after the add/drop deadline. Undergraduates should submit their request to University College for review. Graduate/credential students should submit their request to Graduate Studies & Research for review.
