This is a quick resource form guide for current Nursing students.

• Student Grievance Form

To complete your background and health requirements for the final semester of your nursing program please follow the CastleBranch Instructions.

All health requirements, drug screen, and background checks must be completed and uploaded to the designated site at mycb.castlebranch.com no later than 3 weeks prior to the start of your final semester.

Please refer to the nursing student handbook starting on page 12 for any questions regarding your background check (TB. Immunizations, Insurance, etc.)

The physical exam form is to be completed by your health care provider. You will need a new physical if your last one is over 6 months.

You do not need to purchase malpractice student insurance.
