Defensive Driving Program

Driver Eligibility and Enrollment Requirements

Employees and volunteers must be enrolled and approved in the University Corporation (Corporation) Defensive Driving Program before:

  • Driving a Corporation-owned vehicle
  • Driving a rental vehicle on Corporation business
  • Driving a personal vehicle on Corporation business
  • Requesting mileage reimbursement

To enroll, submit all required documents in one complete packet to corporationdefensivedriving@csumb.edu.

Partial packets will not be processed.

A complete enrollment packet includes Documents 1-5. Document 6 is required only if seeking mileage reimbursement.

Complete packets are required for new drivers. Existing drivers must update expired documents before driving on Corporation business.

All documents have expiration dates. Drivers are responsible for ensuring all documentation remains current.

Document 1 — Application (Signed by Driver)

  • Required for enrollment
  • Expires when the Defensive Driving Certificate expires
  • Must be renewed every four years

Document 2 — Form INF 1101 (Signed by Driver)

Authorization for Release of Driver Record Information

  • Does not expire

Document 3 — Copy of Current Driver’s License

  • Front side only
  • Valid license (any state)
  • Must be resubmitted whenever renewed

Document 4 — DMV Driving Record Printout

  • Must be issued by the state that issued your license
  • Cannot be older than 30 days at time of submission
  • Required at initial enrollment
  • Required for out-of-state drivers

California drivers may request records through the CA DMV Driver Record Request page.

Document 5 — Defensive Driving Course Certificate of Completion

  • Must complete the approved Defensive Driving Course
  • Certificate expires every four years
  • Self-assign the course through SumTotal (instructions available)

Document 6 — Authorization to Drive Privately Owned Vehicle

(Required only if seeking mileage reimbursement.)

  • Signed by driver and supervisor
  • Must be renewed annually
  • Expires one year from supervisor signature date
  • This is the only document requiring supervisor signature
  • Temporary employees may not drive on Corporation business.
  • Volunteers must complete a Volunteer Form through Corporation HR: hr_corporation@csumb.edu

If you are a Corporation employee and need to drive University-owned vehicles, contact the University Defensive Driving Program: defensivedriving@csumb.edu.

Important:

  • The University does not accept Corporation driving documentation
  • The Corporation does accept the University Defensive Driving course

If involved in a car accident while driving on Corporation business:

  • Complete the Automobile Accident Form
