Complete Online SDAC Student Application.

Submitting Disability Verification (DV)

• Disability verification (DV) is NOT required to apply for services.
  • An SDAC Advisor will help you determine the verification documents  needed, based on the CSU Policy.
• If you do have DV, it can be uploaded with your application.
  • Disability Verification should:
    • Be from a licensed treating provider.
    • Explain how your disability/medical condition impacts/causes a barrier to your participation in the classroom or campus housing environments.
    • A diagnosis is not required to receive SDAC services.  
• You may submit your provider's DV form, or have them use one of the forms below:

  • Disability Verification for Psychological Disabilities
  • Disability Verification Related to Housing Accommodations
  • Emotional Support Animal Request Form
  • Expectant Parent/Childbirth Healthcare Form
• To upload DV after submitting your SDAC application
  • Log into the AIM Student Portal and use the "Additional and/or Updated Documentation" feature.  

SDAC ensures confidentiality of all disability-related documents and communication with students.
