Complete Online SDAC Student Application.

Submitting Disability Verification (DV)
- Disability verification (DV) is NOT required to apply for services.
- An SDAC Advisor will help you determine the verification documents needed, based on the CSU Policy.
- If you do have DV, it can be uploaded with your application.
- Disability Verification should:
- Be from a licensed treating provider.
- Explain how your disability/medical condition impacts/causes a barrier to your participation in the classroom or campus housing environments.
- A diagnosis is not required to receive SDAC services.
- Disability Verification should:
- You may submit your provider's DV form, or have them use one of the forms below:

- Disability Verification Related to Housing Accommodations
- Emotional Support Animal Request Form
- Expectant Parent/Childbirth Healthcare Form
- To upload DV after submitting your SDAC application
- Log into the AIM Student Portal and use the "Additional and/or Updated Documentation" feature.
SDAC ensures confidentiality of all disability-related documents and communication with students.
After completing your SDAC student application move on to step 2.
