Online SDAC Student Application.

Submitting Disability Verification (DV)
- Disability verification (DV) is not required to apply for services. Your SDAC Advisor can provide assistance with developing a plan to obtain a comprehensive learning disability assessment or other disability verification, if needed, based on CSU Policy.
- If you do have DV, it can be uploaded with your application. DV should be from a licensed treating provider, and explain how your disability/medical condition impacts/causes a barrier to your participation in the classroom or campus housing environments. Diagnostic information does not need to be included.
- You may submit your provider's DV form, or provide them one of the forms below:

- Disability Verification for Physical/Medical Functional Limitations
- Disability Verification for Psychological Disabilities
- Disability Verification Related to Housing Accommodations
- Emotional Support Animal Request Form
- Expectant Parent/Childbirth Healthcare Form
- If you do not have documentation to upload at the time of completing the application, you can upload it at a later time by logging into the AIM Student Portal and using the "Additional and/or Updated Documentation" feature.
SDAC ensures confidentiality of all disability-related documents and communication with students.
After completing your SDAC student application move on to step #2.
