Welcome Back Otter Returning Student Interest Form

For students who have taken time off from CSUMB and would like to return to complete their degree.  Our "Welcome Back Otter" advisor will contact former students who would like to return to CSUMB.

Reports submitted by faculty/staff

Faculty should use the Academic Early Alert form to report ACADEMIC concerns about a student who has missed class, is not participating in class, or is struggling to be successful in a class. Your report will go to a Student Success Advisor, who will reach out and help connect the student with appropriate resources.

Others (non-faculty) should use the CARE TEAM report below.  

Use this form to report health, safety, or behavior/conduct concerns, or specific incidents of concern. This report goes to the CARE Manager. (EMERGENCIES should be reported by dialing 911). The CARE Manager works closely with Health and Wellness Services, UPD, Student conduct, and other offices to support students in crisis or in difficult personal situations.
