LS ESM CSET Waiver Application Form Step-by-Step Guidelines

The new, revised LS ESM CSET Waiver Application is now available on a google form via Google Suites.

Step 1

Access the LS ESM CSET Waiver Application form using the google form link AFTER reading through the step-by-step application guide.

Step 2 - Name and Contact Information

• Type in your email address (preferred email address)
• Enter your first and last names (YOUR FULL LEGAL NAME please, no nicknames)
  
• Enter your CSUMB student identification number (MUST BE 9 DIGITS - be sure to include zeroes).
  
• Enter CSUMB email address (if same as above, re-enter)
• Enter the phone number where you may be reached if we have any questions about your form (MUST BE 10 DIGITS - include area code).
  
• Type in your full mailing address: (Street Address, City, State, Zip code)
  

Step 3 - CSUMB Entry Status & Application Phase

• Check whether you started as a freshman at CSUMB, or whether you transferred to CSUMB.

• INITIAL REVIEW PHASE - First-time ESM Review
  • Students who started CSUMB as FRESHMAN must submit their Initial Phase application in their SECOND SEMESTER SOPHOMORE YEAR.
    
  • Students who started CSUMB as TRANSFER must submit their Initial Phase application in their FIRST SEMESTER JUNIOR YEAR.
    

• PRELIMINARY REVIEW PHASE - ESM Review for Credential Application
  • Apply at the beginning of the semester that credential application is submitted.
  • If all ESM coursework has been completed, student will receive Final ESM CSET Waiver Letter.
  • If some ESM coursework is pending and not yet completed, student will receive Preliminary Letter.

• FINAL REVIEW PHASE - ESM Review for Credential Admission
  • Apply when ALL ESM coursework has been completed for Final ESM CSET Waiver Letter.

Step 4 - Credential Application Information

• Indicate which type of credential program you plan to apply to at CSUMB (Elementary Education or Special Education).

• Indicate if you plan to apply to a credential program at CSUMB or at another University.

• If you do not plan to apply to a credential program when waiver application is submitted, indicate this option.
• If you have different credential plans, please indicate with OTHER, and briefly describe those plans.

• Indicate the SEMESTER and YEAR you plan to enter the credential program to which you are applying.
  • If not planning to apply to a credential program at the time of the waiver application, write in "N/A."

Step 5 - CBEST Information

• Indicate your current status with CBEST completion.
  • Keep in mind that most credential programs require applicants pass CBEST or an alternative assessment to show Basic Skills competency for admission to credential program.

• CBEST taken and COMPLETELY passed indicates that all three subtests were taken (reading, writing, math), and all were passed.

• CBEST taken and PARTIALLY passed indicates that one or more of the subtests were taken (reading, writing, math), but not all of the three subtests were passed.

• CBEST alternative completed indicates that you are using ACT, SAT,or AP scores as alternates to show Basic Skills competency.

Step 6 - Submit application google form.

Most applications will be immediately processed, and you will receive an automated response indicating which courses have been approved, are pending, declined, or require further review. For courses that require further review by the ESM Review Committee, please allow 2-4 weeks for the course(s) to be fully reviewed. You may be asked to provide information for those courses such as course description, or course syllabus.
